Physicians, parents/caregivers, early interventionists/case managers, and others may send referrals to initiate an evaluation and/or treatment. We will need a completed referral form and a physician's prescription to "evaluate and treat speech-language therapy services and/or occupational therapy services". If you have any additional documentation such as current diagnostic reports, previous therapy evaluations, etc. please include those as well.
